These Terms of Service ("Terms") govern your use of the website and services of Y-Factor Health Inc., doing business as Y-Factor Health. By booking or using our services, you agree to these Terms.

1. Services

Y-Factor Health arranges at-home collection of samples for male fertility testing, laboratory analysis through our lab partner, and physician-reviewed reporting of results.

2. Not Emergency Care

Our services do not provide emergency medical care and are not a substitute for advice from your own physician. If you have a medical emergency, call 911 or seek immediate care.

3. Eligibility

You must be at least 18 years old and able to enter into a binding contract to use our services.

4. Booking, Appointments & Access

You are responsible for providing accurate information and safe, lawful access to the collection location. We may reschedule or decline service where access or safety conditions are not met.

5. Payment, Fees & Insurance

Fees are presented at booking and processed by [PAYMENT PROCESSOR]. You are responsible for all charges. Insurance is accepted only where expressly stated.

6. Cancellations & Rescheduling

Because Y-Factor Health reserves a dedicated mobile laboratory, clinical personnel, equipment, and travel time to provide services at your residence, we require advance notice for appointment changes.

• Cancellations or rescheduling. Appointments may be cancelled or rescheduled at least 48 hours before the scheduled appointment time without penalty.
• Late cancellation or no-show. Appointments cancelled or rescheduled less than 48 hours before the scheduled appointment time, or appointments where the patient is not present or available at the residence at the scheduled time, may be subject to a $150 late-cancellation/no-show fee. If the Y-Factor Health mobile laboratory has already been dispatched to the patient, the appointment may also be treated as a late cancellation or no-show.
• Exceptions. Y-Factor Health may waive the fee for emergencies, severe weather, or other circumstances at its discretion.

Cancellation and no-show fees are administrative charges and are separate from charges for clinical or laboratory services.

7. Test Results

Results are provided for informational purposes and reviewed by a licensed physician. They are not a diagnosis. Discuss results with a qualified healthcare provider before making decisions.

8. Your Responsibilities

• Provide accurate personal and health information.
• Follow all collection and preparation instructions.
• Use the website and services lawfully and not disrupt or misuse them.

9. Intellectual Property

All content on this website is owned by or licensed to Y-Factor Health and may not be copied or reused without permission.

10. Third-Party Services

Laboratory analysis is performed by [LAB PARTNER] and payments by [PAYMENT PROCESSOR], each subject to their own terms.

11. Disclaimers

The website and services are provided "as is" without warranties of any kind to the fullest extent permitted by law.

12. Limitation of Liability
